Technology Services, along with Human Resources, manages the process for requesting systems access. This includes access to the Brown County network, email, various applications (Kronos, Logos, AS/400, etc) and more!
The supervisor of the employee must complete the appropriate form, based on the action needed, AND forward to HR using their own email account. This indicates their approval of the requested access. THESE WORD FORMS WILL BE USED IN THEIR ELECTRONIC FORMAT (Typing your name and forwarding via email is equivalent to a signature). DO NOT PRINT!!.
INSTRUCTIONS: DO NOT SAVE THESE FORMS, they may have changed! Always use the forms from this Intranet page! Complete the form using Microsoft Word, then save the form and forward it to: email@example.com. (File, Save & Send, Send using e-mail, Send as attachment).
Note: Technology Services requires 6 business days upon receipt of this form (minimum) from HR to complete requests. Please allow enough time for processing so the user is not waiting for access.

Use this form when a user has a name change.

USER TRANSFER TO A DIFFERENT DEPT OR WORK UNIT, OR USER JOB CHANGE
Use these forms when a user is transferring to a different department or work unit, or when a user is moving from one position to another (where access to data or systems is changing). IMPORTANT! Both forms are required for a job transfer or job change!!
The supervisor of the transferring out user (or former position if access is changing) must complete the Transfer OUT form below.

USER LEAVE OF ABSENCE (TEMPORARY)
Complete this form when a user is going on a Leave of Absence. This could be for medical, military, administrative, suspension, or any other leave of absence.
